I forgot to mention earlier, but Asm Studio can generate both programs and apps, with build-in headers.

I think latenite can do both apps and multi-page apps relatively painlessly, although I never tried. I also forgot to mention that it is themeable, and has a nice selection of themes (my personal fav is the matrix one, with the classic x86 blue/yellow/purple theme is great for those retro moments )
